What is A.C.E Case Management? 

Access: We will meet with clients at their place of residence. No need to travel to an office. We pride ourselves on being available and accessible to our clients. We are a small case management agency and prioritize our patient relationships. 

Coordinate: We provide case management services within the Wyoming Medicaid’s Community Choices Waiver (CCW) program guidelines. We will assist in coordination of Medicaid services, while also assisting in various case management tasks such as: food stamp applications, LIEAP, Section 8 Housing applications, transportation resources, various community resources and referrals. We assist in coordination of available services within the community. 

Empower: We will provide our clients with the tools and resources to empower themselves to be an advocate for their care. We will be your voice and partner in a system that can be overwhelming at times. We support our clients in meeting their goals and advocating for their independence. We’re there for our clients, providing assistance and encouragement every step of the way.

Meet Alicia

Alicia MacArthur, the owner and creator of A.C.E. Case Management and has been a Social Worker since 1999 years. She has a Master Degree in Social Work from Colorado State University and is a Licensed Clinical Social Worker in Wyoming and Colorado. She has a passion for working with older adults and empowering them to live independently in their own community. She will advocate for the rights and services individuals deserve to live with dignity, respect and autonomy.

 

What We Do

A.C.E. Case Management provides case management services under the Community Choices Waiver (CCW) program which serves individuals who are aged 65 and older, or individuals who are aged 19 – 64 who have a disability that is verified as meeting Social Security Administration (SSA) disability determination criteria.

Wyoming Medicaid’s Community Choices Waiver (CCW) program provides eligible individuals an alternative to nursing facility care through access to an array of community-based services, including:

  • Case Management 

  • Home Health Aide 

  • Personal Support Services 

  • Skilled Nursing 

  • Home-Delivered Meals 

  • Respite 

  • Non-Medical Transportation 

  • Personal Emergency Response Systems (PERS) 

  • Assisted Living Facility 

  • Services

What is MEDICAID?

Medicaid is the state and federal partnership that provides health coverage for selected categories of individuals with low income and limited resources. If you do not have Medicaid, A.C.E. Case Management will assist you in the application process. If you are not Medicaid eligible, there is a private pay option.
